How was it possible, that multiple individuals died, after taking a simple medicine, freely available in a pharmacy?

Was it possible to retrieve the primary root cause of this critical adverse effect?

How quick were the authorities and the firm with their actions and was there a corrective and preventive plan launched?

From a professional point of view, this documentary gives interesting angles to a critical adverse event and the handling of it. 

My deepest sympathies goes out to the families who had and still have to endure this tragedy.
